No, the input and output ordering are not the same.

The input was 3, 6, 4 (see my OP).  In the output, 0, 1, 2 were mapped in the
order 3, 4, 6 (see comment 2).  The result is that the visually impaired audio
track is becoming the first audio track in the output.

I have not tested if "-map 0:p:7:0 -map 0:p:7:1" works, but I did state in the
OP that "-map 0:6 -map 0:3" does work.  This is not a fix for the problem,
however.  This is a temporary work-around.

The fix is to make "-map 0:p:7" maintain the original ordering.
