More information:

pulse origin of time is 0, the other captures use the current time as provided
by av_gettime() and that will cause problems if you try to capture audio from
pulse and video with x11grab.

A patch solving the surface issue landed now, a more thorough approach is
needed though (either document that all the captures should use the same
starting time approach or fix it in libavdevice directly).
