[libav-api] H264 encoded video cannot be played by Windows Media Player 12

Yes, the codec is available in Windows Media Player 12.
The problem seems to be in the generation of the video file.

As stupid as it sounds, since looks like you tried everything else, do you
have the h264 codec that supports the profile you are using?
